Adelante Healthcare offers several ways to schedule an appointment, so it’s easy to access high-quality care when and where you need it. Choose either new patient or current patient below, and book your appointment online or on the phone. Our Patient Support Center is open weekdays 7 a.m. to 6 p.m., and every Saturday 8 a.m. to 5 p.m., and assistance is available in the top 14 languages spoken in Arizona!

The Patient Portal helps you live a healthier life by making your care services more convenient. Sign up and enroll in the Patient Portal to schedule appointments, make payments, receive reminders, access your medical records, and more.
If you need to cancel or reschedule your appointment, please provide at least a 24-hour notice to allow other patients access to your timeslot. If you are unable to let us know at least 24 hours in advance, and we are unable to fill your appointment time, you will be considered a no-show. Please note that if you are a no-show 3 times within 6 months, you will be placed on a standby same-day appointment status for the following 12 months.
A patient is considered late when he or she arrives after their scheduled appointment time. If you arrive late, we will try to work you into the next available time, but other patients who have arrived on time may see the provider first.
Please note that new patients are asked to arrive 30 minutes early in order to complete new-patient forms. Arriving less than 15 minutes prior to your first new patient appointment will be considered late.
