Make the most of your new patient appointment in Arizona

To receive the best possible service and high-quality medical care, be prepared with the items you need to bring, and the patient medical forms our Arizona offices need to access your records. The following tips will help us to help you get the most value out of your visit.


Arrive early

Please plan on arriving 30 minutes before your appointment time to get registered, fill out any required forms, or update your medical records and contact information.


What to bring

Be sure to have the following documents with you when you arrive:

  • Two forms of identification (drivers license, state ID, tribal ID, passport, or government ID)
  • Current health insurance card
  • A list of medications you are taking, including the dosage (or you may bring the actual prescription bottles)
  • A list of other physicians or locations of care you may be currently using
  • Current immunization records
  • Discharge paperwork from hospital visits

Language Assistance

If you or a family member would prefer to receive care in a language other than English, Adelante provides language resources to help. Assistance is available in the top 14 languages spoken in Arizona for patient appointments.


Late arrival

A patient is considered late when he or she arrives after their scheduled appointment time. If you arrive late, we will try to work you into the next available time, but other patients who have arrived on time may see the provider first.

If you are unable to let us know at least 24 hours in advance that you want to cancel an appointment, and we are unable to fill your appointment time, you will be considered a no-show. If you are a no-show 3 times within 6 months, you will be placed on a standby same-day appointment status for the following 12 months.

Please note that as a new patient you are asked to arrive 30 minutes early to our Arizona clinics to complete new patient medical forms. Arriving less than 15 minutes prior to your first new patient appointment will be considered late.
