New patients: Call 480-964-2273 and our Patient Support Center will help you choose a location, find the right service and schedule your first visit.
Returning patients: Book your appointment online or call 480-964-2273. Online scheduling is available for physical exams, sick visits, wellness visits, follow-up appointments and more.
Our Patient Support Center is available:
Monday through Friday: 7 a.m. to 6 p.m.
Saturday: 8 a.m. to 5 p.m.
Language assistance is available. Our team can help connect you with a provider who speaks English, Spanish or another language.
Adelante welcomes AHCCCS members, Medicare patients and uninsured patients. We also accept many commercial insurance plans and offer affordable options for patients who qualify.
Adelante patients can also use Adelante Pharmacy, including free prescription delivery throughout Maricopa County. Our providers and pharmacists work together to help patients understand their medications and stay on track with their care.

If you need to cancel or reschedule your appointment, please provide at least a 24-hour notice to allow other patients access to your timeslot. If you are unable to let us know at least 24 hours in advance, and we are unable to fill your appointment time, you will be considered a no-show.
If you schedule you appointment online you will receive a confirmation email that will have a link to cancel or reschedule your appointment.
A patient is considered late when he or she arrives after their scheduled appointment time. If you arrive late, we will try to work you into the next available time, but other patients who have arrived on time may see the provider first.
Please note that new patients are asked to arrive 30 minutes early in order to complete new-patient forms. Arriving less than 15 minutes prior to your first new patient appointment will be considered late.
