We welcome most insurance plans including AHCCCS and Medicare.
When you call to schedule your appointment, or Patient Support Representative will verify your insurance and confirm that it is still accepted. You will be asked to provide a current insurance card when you check in for your appointment. Please arrive 15-20 minutes early to allow us time to update your records.
Adelante Healthcare offers an affordable self-pay option for patients who are uninsured or underinsured and may not qualify for financial assistance.
The initial doctor’s visit will be between $100 and $250 (plus any additional services required). Subsequent appointments are $80 to $180 (plus any additional services which may be required). These additional services include labs, in-office medications which are administered by your provider or Medical Assistant, or other procedures performed while you are in the office. You will receive a statement listing the charges for the visit plus these additional charges shortly after your appointment.
If you are uninsured or underinsured, you may qualify for programs and services with reduced fees based on income and family size including the State’s AHCCCS Program or Adelante Healthcare’s Sliding Fee Program. Learn more about these options.
Your copay or appointment fee is due on the day of your appointment prior to seeing the doctor.
If you are on the self-pay or sliding fee program, payment for those services is also due the day of your appointment.
In some cases, you will receive notification of a balance due after the initial appointment after insurance has been submitted or if additional services such as labs, radiology, etc. are needed.
We accept cash, checks, Visa and MasterCard credit and debit cards.
If there is an outstanding balance on your account, we may ask you to pay it in full or set up a payment plan in order to schedule future appointments.
This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
