Healthy living begins with a healthy diet. That’s why Adelante Healthcare works with the Women, Infants & Children (WIC) Program. In Phoenix, and throughout the rest of the US, WIC helps those who are pregnant, breastfeeding, and their children under age 5 eat good-for-you foods, at no cost!

Who qualifies for WIC?

Throughout the United States (Including Phoenix, AZ) the WIC program provides nutrition and health education, healthy food, and other services free to families who qualify. You can participate in WIC if you live in Arizona, have a nutritional need (our staff can help you determine this), and you are one of the following:

• Pregnant
• Have delivered a child within the last six months
• Breastfeeding a child within the last year
• Infants and children under the age of five, including foster children

What does WIC offer?

• Nutrition education
• Breastfeeding support
• Experts who specialize in nutrition for mothers and their children
• EWIC card to buy healthy food
• Referrals to health care, immunizations and other community services

Applying for WIC is easy. Simply submit the necessary documents to get started. Here is what you’ll need:

• Your driver’s license or photo ID
• Identification for everyone enrolling in the program. For children, that may be a birth certificate, crib card, or shot card/immunization record
• Proof of address (ex. electric bill)
• Proof of income (ex. pay stub or letter from AHCCCS or SNAP)

Adelante Healthcare in Phoenix offers WIC program services when and where you need them, with a variety of access options to suit your needs and lifestyle. Same-day services are available at your closest Adelante Healthcare referral center in Phoenix or the surrounding communities. Our offices provide phone and virtual appointments, as needed.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
