In Mesa, embracing a lifestyle of health and wellness starts with the foundational element of a nutritious diet. Eligibility for WIC in Mesa:

Eligibility for WIC is determined based on residency in Arizona, a verified nutritional need (our dedicated staff are here to assist in this evaluation), and falling into one of these categories:

  • Infants and children under the age of five, including foster children

Applying for WIC is easy. Simply submit the necessary documents to get started. Here is what you’ll need:

  • Your driver’s license or photo ID
  • Identification for everyone enrolling in the program. For children, that may be a birth certificate, crib card, or shot card/immunization record
  • Proof of address (ex. electric bill)
  • Proof of income (ex. pay stub or letter from AHCCCS or SNAP)
