How to redeem your Free Backpack Voucher:

Schedule a Well-Child Check between July 15th, 2024 – August 16th, 2024. Appointments can be scheduled online or by calling 480-964-CARE.
Click the Free Backpack Voucher button below and complete the form.
After you hit “Submit” your voucher will appear, screenshot or download the voucher to your device.
After you have completed your well-child check appointment, present the voucher to the front desk to redeem your free backpack. You can do this by showing them from your device, printing out voucher, or however works best for you.

Terms and conditions apply. While supplies last. 1 backpack per school-aged child that completes a well-child check. Only valid between the dates of July 15th, 2024 – August 16th, 2024. This voucher does not guarantee a backpack.
