We want to provide all of our patients with good customer service while making sure that each receives the medical attention they need to be well. The following tips will help us help you while also ensuring that we can help others as well.

If this is your first visit, you can save time before your appointment by printing and completing the following Patient Forms in the comfort of your own home:
If you have any questions about any of the Adelante Healthcare forms, please contact us at 1-877-809-5092.
If you or a family member need would prefer to receive care in another language, Adelante provides language resources to help. Just let your care team or the front desk know.
A patient is considered late when he or she arrives after their scheduled appointment time.
If you arrive late for your appointment, we will try to work you into the next available time but other patients who have arrived on time for their appointments may see the provider first.
If you are unable to let us know at least 24 hours in advance that you want to cancel an appointment and we are unable to fill the appointment, you will be considered a “No Show.” (Note that if you are considered a “No Show” for your appointment 3 times in 6 month, you will be placed on standby same-day appointment status for the next 12 months.)
Please note that new patients are asked to arrive 15 minutes early in order to complete new-patient forms. In that case, a late arrival would be less than 15 minutes prior to the appointment time.
