What to Bring With You

Please plan to arrive 15 to 20 minutes before your scheduled appointment time so that we can update your medical records.

For Your Medical Appointment
We will need you to bring:

  • Two (2) forms of ID
  • Your insurance card
  • List of prescriptions and current medication bottles
  • Current immunization records
  • Any discharge paperwork from any hospital visits

For Your Eligibility Appointment
We will need you to bring:

  • Proof of Current Residency (No PO BOX)
  • Identification
  • Proof of Citizenship/U.S. Citizen
  • Social Security Cards/Numbers
  • Telephone number
  • Proof of All Income from Any Source for Last and Current Month
  • Family size
  • Proof of Pregnancy with Estimated Date of Delivery
  • Medical Expenses for Last and Current Month
  • Medical Insurance Cards, including AHCCCS cards

Find a Doctor

Our staff of highly skilled physicians provides quality care across a wide range of service areas. We offer services such as immunizations, pediatrics, prenatal care, dentistry and more to get you healthy and keep you that way. We have doctors at our centers who will meet your needs.

Make an Appointment

Once you’ve located your center and chosen the doctor you’d like to see, contact us at     1-877-809-5092 to schedule an appointment. Attending to your needs in a timely manner is important to us, so appointments often are available later the same day or within 1 to 2 days. If you must cancel or reschedule, please provide us with at least 24 hours notice. Walk-in patients may be seen, depending on doctor availability.

Prescriptions

For refills, please call your pharmacy at least three days before your medication runs out. Bring your medications to every appointment so that we can review them for any changes or possible drug interactions.

We are unable to schedule appointments or refill medications after hours.

After Hours Care

For urgent medical problems after hours, call 1-877-809-5092 to leave a message for the doctor on call. Dial 911 for life-threatening emergencies.
